Background
The Discharge Medicines Service (DMS) is an essential service that pharmacy contractors are required to provide. NHS Trusts refer patients to their community pharmacy if they find that they may benefit from having extra support and guidance on changes to their medication.

Recording DMS in PSC
Counselling Notes
One option is to use the Counselling Notes feature to record details for each stage of the service. For example:
Create a new counselling note for each stage:
Warning Notes
On accessing a patient’s Patient Details window, you can create a warning note (red tag); this is found in the bottom-right hand corner of the window. The warning note will be shown intrusively when the PMR is first accessed.
Add the details that Stage needs to be completed and remove it once it has been carried out:
This will help your staff to remember to conduct the next stages of the DMS service.
Interventions Module
The Interventions module is another option to record the specific discussions taken place between the pharmacist/pharmacy technician and the patient and/or their carer. This can be recorded under Prescription Intervention:
