Printing NMS forms

GP Referral form

When performing an NMS intervention or follow up, you may want to refer the patient back to their GP for further investigation. Within ProScript Connect, you can print a GP referral form to provide the GP with your observations.

Tip: You can also print the GP referral form from within the NMS record. The option, F8 – Print GP Referral is however only active, if the Referral section within the Intervention or the Follow Up questionnaire has been completed.

  1. Whilst performing an Intervention or Follow Up Questionnaire, navigate to the Outcome of the discussion with patient tab.Intervention Questionnaire
  2. Select Patient’s issues raised with the new medicine need to be considered by the prescriber tick box.
  3. Select the reason for the referral using one or more of the tick boxes within the Referral section, and add any notes into the Other field, if required.
  4. Once the Referral section has been completed, click F8 – Print GP Referral Form button.
  5. The Report Preview is displayed, click the printer icon to print the form.

NMS form for a selected patient

NMS forms can be printed/exported for a selected patient or as a blank form.

  1. From the ProScript Connect Main Screen, click ProScript Connect, then select NMS Manager.NMS Manager
  2. From the General tab, use the filters to locate the relevant NMS record.
  3. Click F9 – Print NMS Form, then select Print NMS for selected patient.NMS record
  4. On the Report Viewer screen, use the printer icon to print the report.

Blank NMS form

  1. From the ProScript Connect Main Screen, click ProScript Connect, then select NMS Manager.
  2. From the General tab, click F9 – Print NMS Form, then select Print Blank NMS Form.Blank NMS Form
  3. On the Report Viewer screen, use the printer icon to print the report.
